COVID-19 TESTING AND VACCINATION: NO APPOINTMENT NEEDED

The Florida Department of Health in Putnam County (DOH-Putnam) is offering free COVID-19 testing and vaccinations, with no appointment necessary.
Testing Location and Times:
Teacher Training Center
1101 Husson Avenue, Palatka
Every Monday through Thursday, 9 am - 2 pm
Walk-up testing
Vaccination Site and Times:
Putnam County Health Department
2801 Kennedy Street, Palatka
Monday- Friday, 9 am – 3 pm
Walk-up
Moderna Vaccine is available for those 18+
Pfizer Vaccine for those 12 +
Johnson & Johnson 18 +
For residents who are under 18 years old, DOH-Putnam is offering only the Pfizer vaccine. For all individuals under 18, a parent/guardian must be present at the time of vaccination and sign the COVID-19 consent forms.
To expedite visits, participants should download and fill out the appropriate consent form and bring it with them to this event. For those who are unable to download this form and print it out, forms will be available at the event. Read and review the appropriate Fact Sheet and Notice of Privacy Practices.
