Putnam Attends NAACHO

Palatka, Fla. - Christiana Daley, Health Educator attended the 2019 NAACHO Conference in Orlando on DOH-Putnam's Chronic Disease Program.

The Model Practice Award Winner poster "Implementing Lifestyle Changes for Chronic Disease Management and Prevention Through Education and Community Support" highlighted the program's successes and challenges.

The program was developed and implemented with the use of a Quality Improvement Plan to increase recruitment and awareness of the DOH-Putnam Heart Health Program, Diabetes Prevention Program and Diabetes Self Management Program to the residents of Putnam County and increase our community partnerships.

DOH-Putnam's Chronic Disease Prevention Programs aim is to improve the health of Putnam County residents through education and support to avoid or manage many chronic diseases that have affected the people of Putnam County.
