DOH-Putnam would like to invite you to take advantage of two programs we are now offering to assist you in taking control of your health and wellness.
One class is a diabetes prevention and the second class is for those who have already been diagnosed with diabetes. Both classes are taught by a registered dietician and certified health coach.
Please share the following information with family, friends and co-workers who are seeking a program to implement lifestyle modifications toward a healthier lifestyle.
Diabetes Self-Management Education Program
This is a 6-week program to help you develop skills to manage and control diabetes. A referral from your physician is required.

Diabetes Support Group
Offered the second Wednesday of every month at 3 p.m., at the Florida Department of Health in Putnam County 2801 Kennedy Street.
We also have a monthly support group on the second Wednesday of the month in Melrose at 10 a.m. Additional support groups will be offered soon.

Blood Pressure Education Classes
Concerned about your blood pressure and not sure what to do to control it, the Health Heart Ambassador Class gives you the skills to make small lifestyle changes that can assist is maintain healthy blood pressure reading.
