Diabetes Prevention and Management

Florida Department of Health in Putnam County

The Florida Department of Health in Putnam County (DOH-Putnam) would like to invite you to take advantage of two programs we are now offering to assist you in taking control of your health and wellness.

One class is a diabetes prevention and the second class is for those who have already been diagnosed with diabetes. Both classes are taught by a registered dietician and certified health coach.

Please share the following information with family, friends and co-workers who are seeking a program to implement lifestyle modifications toward a healthier lifestyle.

Beat Prediabetes

Diabetes Self-Management Education Program

This is a 6-week program to help you develop skills to manage and control diabetes. A referral from your physician is required.
