Community Event Request

To Request Participation in an Event: 

The Florida Department of Health in Putnam County participates in a variety of health fairs and community events throughout the year to promote and support overall community health. To request the Florida Department of Health in Putnam County to participate in your community event, please contact Cathy Horton at 386-326-3222.

When making the request, please note the following:

• Participation in community outreach events is based on staff availability and focus area of our programs
• Requests must be made 2 weeks prior to your event
