Diabetes Prevention and Management

Florida Department of Health in Putnam County

DOH-Putnam would like to invite you to take advantage of two programs we are now offering to assist you in taking control of your health and wellness.

One class is a diabetes prevention and the second class is for those who have already been diagnosed with diabetes. Both classes are taught by a Registered Dietician and Certified Health Coach.

Please share the following information with family, friends and co-workers who are seeking a program to implement lifestyle modifications toward a healthier lifestyle.

Beat Prediabetes

Diabetes Self -Management Education(DSME):

This is a 6-week program to help you develop skills to manage and control diabetes. A referral from your physician is required. Please call 386-326-3223 for more information.

Is Diabetes Controling your life?Prevent T2 Diabetes:

If you have prediabetes or other risk factors for type 2 diabetes, it is time to take charge of your health. The prevent T2 lifestyle change program can help you make lasting changes to prevent type 2 diabetes. Please call 386-326-3277 or 386-326-3223 for more information.

Diabetes Support Group:

This is offered the second Wednesday of every month at 3pm. It is located currently at the Florida Department of Health in Putnam County 2801 Kennedy Street. We also have a monthly support group on the second Wednesday of the month in Melrose at 10 AM. Additional support groups will be offered soon. Please call 386-326-3223 for more information.

Blood Pressure Education Classes:

Concerned about your blood pressure and not sure what to do to control it, this class gives you the skills to make small lifestyle changes that can assist is maintain healthy blood pressure reading. Please call 386-326-3277 for more information.
