Healthy Families Closure

Since 1999, the Healthy Families Putnam program has been housed within the Florida Department of Health in Putnam County to support families with the resources, guidance, and care needed to build safe, stable, and nurturing homes. Due to reductions in statewide Healthy Families funding, the Healthy Families Putnam program will close on September 30.

Our priority during this transition is to help families maintain continuity of care. To ensure families are linked to the best available support, we are referring them to CONNECT-NE Florida Healthy Start Coalition. During this transition, our DOH-Putnam staff will remain available to assist families, answer questions, and coordinate care to the best of our ability.
