The Florida Department of Health in Putnam County offers dental services for both children and adults. We pride ourselves on having a friendly and positive dental environment while educating patients on good oral hygiene. We are open Monday through Friday from 8 a.m. to 5 p.m. We accept Florida Healthy Kids (FHK), Liberty Dental, DentaQuest and MCNA insurances.

- Proof of Legal guardianship/custody (if you are not the legal parent)
- If requesting Sliding Fee Scale services, verification of income will be required
