Prevention and Management
If you have prediabetes or other risk factors for type 2 diabetes, it is time to take charge of your health.
Taught by a registered dietician and certified health coach, learn about our lifestyle change program to help you build healthy new habits that last a lifetime and ways to reduce your risk of developing diabetes.
When you join a lifestyle change program, you will learn, laugh, share stories, try new things and build new habits.

Prevent T2 Diabetes

Self-Management Education
Taught by a registered dietician and certified health coach, the Diabetes Self-Management Education Program is a 6-week program to help you develop skills to manage and control diabetes.
A referral from your physician is required.

Blood Pressure Support & Education Classes
Concerned about your blood pressure and not sure what to do to control it?
Our Health Heart Ambassador Class gives you the skills to make small lifestyle changes that can assist is maintain healthy blood pressure reading.
